Mediation Documents

There are remarkably few forms associated with CORE mediations. All are standard throughout the DOI CORE program:

  • The Intake Form (PDF or Word doc) is completed by the CORE Specialist and signed by the person requesting CORE mediation services. A copy of it is retained by the USGS CORE Manager. A principal purpose of this form is to document that individuals are informed and aware of their rights under the EEO complaints process.
  • The Agreement to Mediate (PDF or Word doc) is signed by all parties participating in a CORE mediation, including the mediators. It describes the basic terms governing the mediation process.
  • The Resolution Agreement (PDF or Word doc) documents in very specific terms the agreements reached by the parties during mediation. It is signed by the parties, as it is their agreement. If any of the terms of the agreement require the review or approval of anyone other than the mediating parties, such as the HR Office or a higher level manager, additional signatures may be necessary.
  • The Notice of Results and Options (PDF or Word doc) is prepared by the CORE Specialist when a mediation does not result in resolution and a signed agreement. It documents that the initiating party has been advised of his/her options for pursuing resolution or closure.
