Voluntary Leave Transfer Program

The Voluntary Leave Transfer Program allows employees to donate annual leave to other USGS employees (excluding the leave donor's immediate supervisor) and employees of most Federal agencies who have a personal or family medical emergency and have exhausted their available paid leave.

Forms

To apply to become a leave transfer recipient, use form OPM 630, Application to Become a Leave Recipient Under the Voluntary Leave Transfer Program.

To donate annual leave to a USGS leave transfer recipient, use form OPM 630-A, Request to Donate Annual Leave to Leave Recipient Under the Voluntary Leave Transfer Program (Within Agency).

To donate leave to a leave transfer recipient at another Federal agency, use form OPM 630-B, Request to Donate Annual Leave to Leave Recipient Under the Voluntary Leave Transfer Program (Outside Agency).
