USGS 445-2-H, Summary of Fiscal Year Changes

FY 2002 (October – September):

1.CH. 24, Occupational Medicine – Lyme Disease Protection Program (Revised 04/02 to eliminate Lyme Vaccine Requirements).

2. CH 41, Cableways (Revised 07/02 to add requirements for removing cableways from service, removing bridge measurements requirements (Moved to Chapter 17-Work Zone Safety), Update Appendix 41-1-1)

3. CH. 42, Electrofishing, was revised 09/02 to incorporate changes to paragraph 42.4.L. Concerning when a PFD must be worn and what type.

4. CH 23, Medical Monitoring Protection Program, was modified to include the offering of Hepatitis A vaccine to employees working in polluted waters (23.5 C 2). Also added were the need to determine the feasibility and legality of providing of medical monitoring to contract employees whenever a new contract is considered (23.4 A). The statement that medical monitoring is mandatory for employees when required by law and is a condition of employment was also added (23.4A). Added Appendix A 23-1; A 23-2; A 23-3; and A 23-4. (Revised 09/22)

5. CH. 14, Safety and Health Training, added Appendix 14-2, Master Training Requirements. (Revised 09/2003)

FY 2004 (1st Quarter):

6. Chapter 7, Modified "Incident/Accident Reporting and Serious Investigation" Under 7.1.A Changed Property Loss Reporting Requirement from 250,000 to 100,000. Under 7.3.B added the OWCP CA-1 and CA-2 Reporting Requirements(Revised 10/03)

7. Chapter 9, Modified "OSHE Councils, Committees and Working Groups" Added a section that defined acronyms. Throughout the document added the term occupational when referring to the USGS OSHE Council (Revised 10/03)

8. Chapter 25, Modfied "Occupational Medicine - Blood borne Pathogens Program" Under 25.3.C added examples of USGS Occupational Exposures. Under 25.4.A added a template for General Exposure Plan. Under 25.5.C.E added the requirement for Organizational Managers and Supervisors the requirement to review exposure control plans annually..  Made modifications to the definition section.(Revised 10/03)

9. Chapter 39, Modified "Wildlife Netting Safety" - Newly Added Explosive Criteria.  Added Department of Transportation Hazardous Materials Class 1 Explosive Divisions (Revised 10/03)

FY 2004 (2nd Quarter):

10. Chapters 2,3,5, and 6. References to the Army Corps of Engineer's WebCas (incorrectly named WEBCASS) deleted due to discontinuance of the system

11. Appendix 3-1 Fiscal Year Safety and Environmental Management Action Plans: Under Strategic Plan Goals 1-5, replaced existing table with a new table containing the following elements: ( Responsible Office; Initiative Description; Management or Regulatory Practice; Primary Program Elements; Secondary Program Elements; and Estimated Pay Periods) (Revised 04/04)

12. Appendix 3-2 Fiscal Year Safety and Environmental Program Accomplishments: Under Strategic Plan Goals 1-5, replaced existing table with a new table containing the following elements: (Responsible Official; Action Item Description; Regulatory or Management Practice; Estimated Pay Periods; Estimated Costs; and Priority 105) (Revised 04/04)

13. Chapter 2.7; 2.10; and 2.11 Changes in section ordering due to deletions of the Army Corps of Engineer's WebCas (incorrectly named WEBCASS)

FY 2004 (4th Quarter):

14. Chapter 26, hyper linked several references A-D and F-I (Revised 08/04)

15. Chapter 26, deleted reference to SM 403.26, Prescription Glasses or Goggles. This chapter is now obsolete. (Revised 08/04)

16. Chapter 26, added references H and I (Revised 08/04)

17. Chapter 26, replaced 26.4.I title "Other Equipment" and it's contents with " Protective Equipment may be furnished when the items are..." (Revised 08/04)

18. Chapter 16, Modified SM 445-2-H, Chapter 16.3E(c) to reflect policy on ATV's. (Revised 08/04)

FY 2005 (2nd Quarter):
