Bird Banding Laboratory Data Manager Request form

Form to be completed by a Master Permitee, or Responsible Individual of a Master Station Permit, to request a Data Manager be added to their permit and to have access to the Bander Portal in order to submit/modify data to the U.S. Geological Survey, Bird Banding Laboratory. Once completed, the form needs to be emailed to bbl_permits@usgs.gov.
