  1. Please contact the laboratory prior to shipping or delivery of samples to arrange for receipt and proper storage.
     
  2. Please type and use the shipping label, chain of custody, and sample data forms provided (see related files at right) for all samples being analyzed.
     
  3. Samples may be refrigerated, but DO NOT FREEZE samples or allow samples to become frozen during shipment. A freeze indicator can be included in the sample crate if shipping from a cold environment where this may be a concern.
     
  4. Please ship samples in a padded crate. Remaining sample can be returned in its original container upon completion of analysis if a FedEx or equivalent shipping account number is provided; otherwise, remaining samples will be discarded 90 days after completion of analysis unless arrangements are made for pickup.
